When signal quality makes all the difference."},"content":{"rendered":"<p>In clinical practice, the electrocardiogram is often regarded as a standard routine test. This view, however, reduces the ECG to its final output \u2013 the printed trace \u2013 whilst overlooking the technical complexity that determines the signal\u2019s <strong>actual diagnostic reliability<\/strong>.<\/p>\n<p>The clinical value of an electrocardiogram depends on the robustness of the entire signal acquisition and management chain. This extremely small scale makes the signal inherently susceptible to interference and distortion along the acquisition chain.<\/p>\n<p>Environmental disturbances, problems with electrode-skin contact, positioning errors or inappropriate filter settings can significantly alter the ECG trace, even in the absence of any actual cardiac pathology.<\/p>\n<p>Even before clinical interpretation, the question of measurement reliability arises<strong><em>: how reliable is the recorded ECG signal?<\/em><\/strong><\/p>\n<p>The quality of the trace depends on:<\/p>\n<p><strong>1. The skin-electrode interface<\/strong>. Contact impedance, skin preparation, gel quality and electrode stability directly affect the signal.<strong>\u00a0Flawed<\/strong> handling introduces noise and baseline instability.<\/p>\n<p><strong>2. A technically reliable ECG directly influences:<\/p>\n<ol>\n<li><strong>Diagnostic accuracy<\/strong>: it reduces the need for secondary investigations (echocardiograms, stress tests, diagnostic coronary angiography) and unnecessary hospital admissions.<\/li>\n<li><strong>Reduction in time-to-therapy<\/strong>: in emergency situations, the quality of the trace directly influences the speed of clinical decision-making.<\/li>\n<li><strong>Quality of follow-up<\/strong>: the value of the ECG over time depends on the ability to make consistent longitudinal comparisons.<\/li>\n<\/ol>\n<p>To ensure these standards are met, the signal must be stable, clean and free from significant artefacts. Advanced ECG systems incorporate <strong>Signal Quality Assessment (SQA<\/strong>) algorithms that analyse signal stability in real time, identify acquisition anomalies and classify the trace as acceptable or unacceptable.<\/p>\n<p>This type of support reduces reliance on operator experience and improves standardisation.<\/p>\n<h2>The ECG today: from a snapshot to longitudinal data<\/h2>\n<p>Traditionally, an electrocardiogram provides a \u2018snapshot\u2019 of the heart\u2019s electrical activity at a specific moment. Its clinical value increases when it is used as part of a comparative <strong>monitoring process<\/strong>, allowing data to be compared over time.<\/p>\n<p>This approach requires consistency throughout the entire data acquisition process: the same <strong>technique<\/strong>, the same lead <strong>configuration<\/strong>, <strong>structured data storage<\/strong> and the ability to analyse <strong>trends<\/strong>.<\/p>\n<p>Essential in clinical care for:<\/p>\n<ul>\n<li>monitoring cardiac arrhythmias<\/li>\n<li>tracking the progression of conduction disorders<\/li>\n<li>ischeaemic follow-up<\/li>\n<li>detecting and screening for silent arrhythmias<\/li>\n<\/ul>\n<p>Monitoring <strong>atrial fibrillation<\/strong> enables the identification of arrhythmic episodes, including silent ones and the prompt initiation of treatment <strong>strategies that reduce the risk of stroke<\/strong>.<\/p>\n<p>To summarise this paradigm shift: an ECG is not an image, but a piece of<strong> data. <\/strong>And data is only valuable if it is<strong> reproducible and comparable.<\/strong><\/p>\n<h3>Integration with new technologies: opportunities and limitations<\/h3>\n<p>Today, the integration of professional devices with new technological frontiers (wearables, ECG patches) is expanding the scope of continuous monitoring.<\/p>\n<p>The Apple Heart Study (NEJM) has demonstrated that notification algorithms can identify rhythm anomalies\u00b2. It is more useful to consider data quality and the reduction of operational risk.<\/p>\n<p>Three questions can guide the choice:<\/p>\n<ol>\n<li>Does the device guarantee <strong>stable, clear tracings<\/strong> even under less-than-ideal conditions?<\/li>\n<li>Does the system allow for <strong>structured storage<\/strong> and <strong>reliable comparison<\/strong> with previous examinations?<\/li>\n<li>Does the software support the operator in <strong>reducing variability and the risk of human error<\/strong>?<\/li>\n<\/ol>\n<h2>The diagnostic value of an ECG depends on the quality of the signal<\/h2>\n<p>Reducing an <strong>electrocardiogram<\/strong> to a printed output means losing sight of its true clinical value.<\/p>\n<p>An ECG is the result of a complex measurement process, in which every stage contributes to the reliability of the diagnosis.
